چکیده

In certain circumstances the discharge rate of the A-V node may exceed that of the S-A node. The A-V node then becomes the dominant pacemaker of the heart. If, in addition, there is retrograde unidirectional A-V block, the atria remain under control of the S-A node, and A-V dissociation results. When a sinus impulse reaches the A-V node during the non-refractory phase, it is conducted to, and captures control of, the ventricles for one beat-known as a capture beat; occasionally 2, or rarely 3, consecutive capture beats occur. An analogous situation may arise when the ectopic pacemaker is situated in the ventricles. This communication reports further observations on A-V nodal conduction in A-V dissociation, with special reference to conduction in consecutive capture beats.

A Comparative Analysis of Antegrade and Retrograde Conduction Patterns in Man By MASOOD AKHTAR, M.D., ANTHONY

Patterns of antegrade and retrograde conduction and refractory periods were studied using His bundle electrogram recordings, incremental atrial and ventricular pacing and the extrastimulus technique. In 36/50 patients antegrade conduction was "better" than retrograde conduction (group I), as evidenced by a) onset of retrograde atrioventricular (A-V) nodal Wenckebach phenomenon at a slower rate ...

Studies on ventriculo-atrial conduction and the reentry phenomenon.

SUMMARY Ventriculo-atrial conduction and the reentry phenomenon was studied during right ventricular and bundle of His pacing in the intact dog heart. Bipolar electrogram recordings were obtained from the sinus node, Bachman's bundle, the right and left atria, the bundle of His, and the ventricle. Retrograde conduction resulted in a specific change in the sequence of atrial activation. The reco...

Retrograde ventriculoatrial conduction in patients with acute myocardial infarction.

Temporary transvenous pacemakers were inserted in ten patients with acute myocardial infarction complicated by atrioventricubu (A-V) block. With return of sinus rhythm, the ventricular rate was intentionally increased above sinus rate by graded increases of the pacemaker rate and stimulus amplitude to a maximum of 120 beats/minute and to 12 milliamperes for each paced rate. Four patients had ve...

Supraventricular tachycardia initiated by sinus beats.

In the present report, A-V dissociation with concealed retrograde conduction into the A-V node, caused SINUS beats to exhibit antegrade A-V nodal conduction delay sufficient to result in episodes of supraventricular tachycardia (SVT). In this patient, atrial ectopic premature depolarizations were never observed. The primary role of A-V nodal conduction delay in the genesis of paroxysmal SVT is ...
